Negligence by HDFC bank agent

Please be informed that my Husband is already holding an account with HDFC bank. So even i decided to open up the account with HDFC bank. So, i had asked the bank representative to visit home for document collection. The agent, Sxxxy collected all of the relevant documents including my photographs. A day later my husband tried calling Sxxxxxy as he had a query however his number was not rechable. A bank represetative (Sxxxxxxxa Rxxxxxxxxxt) called up my husband and said that photographs have been misplaced (either we have not given or agent must have lost it). My Husband insisted that the agent has lost the photograph. We had handed every document when Sxxxxxy had collected else he would have not given the kit.

I need to lodge a complaint against such a careless act where confidential documents are handled carelessly. This is a sensitive issue. Please help.
