Facing issues in medical claim reimbursement

I underwent black fungus treatment in RML Hospital New Delhi, after discharge doctor advice to take four week medicine from home which is written on my discharge summary, I followed the doctor advice and continued my treatment accordingly after that I submitted my mediclaim form duly signed by the doctor along with all necessary paper in original but TPA team has raised query , I reverted on the query , but they again raised query in this way they are trying to trouble me so I can close my case

Refund from Flipkart – Rs.5,999/-: Mr. Dinga Sailo of Aizawl 796001, Mizoram ordered a mobile handset from Flipkart.com on 6th January 2015 for Rs.5999/-. Kotak Mahindra Bank changes a/c type without information – deduct charges from account: Mr. Atul Jain of Indore had posted a complaint on 03-09-2009 in the Forum with regard to Kotak Mahindra Bank changing his account type and debiting his account for non-maintenance of average quarterly balance.
